- To commemorate the services performed by the personnel of the Navy and Marine Corps during the operations in Nicaragua between 27 August 1926 and 2 January 1933.
- The award was made to the officers and enlisted men of the Navy and Marine Corps who served on shore in Nicaragua between the dates mentioned.
- The award was made to the officers and enlisted men who were attached to and serving onboard the vessels mentioned in the following list, between the dates set below each vessel.

- Columbia is armed with a sword.
- Columbia is defending two other figures with her cloak.
- Arced above Columbia are the words "SECOND NICARAGUAN CAMPAIGN".
- Below are the the dates "1926 – 1930".
- Displays Columbia, the female national personification of the United States.
- REVERSE
- Both versions have an eagle perched on a horizontal ship's anchor.
- Arched above the eagle are the words "UNITED STATES NAVY" or "UNITED STATES MARINE CORPS" depending on the service.
- In the middle are the words "FOR SERVICE".
- Interwined with the anchor are branches of laurel.
- RIBBON
